PATIENT PORTAL

We invite all patients to connect with us on our new patient portal! You may use this link if you have ALREADY established a portal with us. If this is your first time, please contact the office to get an invitation to the portal to set one up, otherwise you will set up a link that will not link to your child's chart. The patient portal is also a secure way to email NON-URGENT messages to your child's physician. If you need to speak with someone urgently or need to schedule an appointment, please call the office. 

CHADIS Chadis is an online system that will deliver developmental and health questionnaires directly to parents and families. CHADIS results come directly to your doctor at CAHC. Use CHADIS for all visits and all ages. It will help your doctor at CAHC address any concerns about your child's health and development. TRAVELING?

Please check www.cdc.gov/travel  for any travel advisories. Simply choose the country you will be visiting and the recommendations will be listed including up to the minute advisories. Some travel vaccines can be obtained in our office and some need to be given at a travel health clinic. PLEASE check weeks to months before travel so that your child and family can be adequately protected.

Dr. Joe Avni-Singer formed CAHC with Dr. Deborah Ferholt in 1999. He grew up in Albany, NY, received a B.S. from Cornell University and attended New York University School of Medicine. He came to New Haven in 1989 for residency in Pediatrics at Yale, and went on to complete a fellowship in Developmental-Behavioral Pediatrics at Yale as well. After working for several years at the Fair Haven Community Health Center, he joined Dr. Ferholt. He found that she had created a unique practice where doctors take time to get to know each child and family individually and provide care with patience, and respect. Though the practice has grown, Dr. Avni-Singer and his partners strive to maintain these core values and protect the time they spend with patients and families. In addition to addressing preventive care and managing children with illness, he finds great fulfillment in helping parents with the challenges of raising families in our complicated world. Dr. Avni-Singer lives in New Haven with his wife and three children, of whom 2 are now in college. 

 

 

 

Dr. Shari Storeygard joined CAHC in 2006. She grew up in the Philadelphia area in a big family of educators. She attended Haverford College and then earned her medical degree at The George Washington University. Dr. Storeygard completed her pediatric residency training at Yale-New Haven Children's Hospital. She lives with her husband and three little boys in Woodbridge. For fun and relaxation, she loves to hike and spend time outside with her family, cook, and travel.

 

 

 

 

 

 

Dr. Shannon Martinello joined CAHC in 2009 after practicing in Fairfield County for 11 years. She is a Connecticut native who attended Providence College and then Loyola University Chicago Stritch School of Medicine. She completed her pediatric internship, residency and an additional year of training in Developmental Pediatrics at  Riley Hospital for Children, Indiana University Medical Center in Indianapolis. She lives in Woodbridge with her husband and four boys. Her area of interest is children with special healthcare needs. She enjoys going to the beach, tennis, traveling and reading when she is not cheering on her boys at their sporting/music/school events.

Dr. Miranda Bik-Yin Ip is the newest pediatrician to join CAHC in 2015. Originally from Massachusetts, she moved to the west coast to attend Stanford University where she earned a degree in biology. She spent a year in Americorps before earning a Masters degree in Health and Medical Sciencesfrom UC-Berkeley. She earned her medical degree from University of California, San Francisco and completed her pediatrics residency at Massachusetts General Hospital for Children. Her areas of interest include newborn care, obesity and asthma. She lives in New Haven with her husband and daughters. She enjoys running, cooking, playing the piano and reading.
